I agree to participate in the data collection that is offered through Go for 8 Wellness Program which includes county of residence, height, weight, and minutes of exercise. I agree that Le Bonheur Community Health & Well Being may use information about my progress for data collection and reporting purposes. I understand that all information collected is confidential, and that my name and other identifying information will not be used in conjunction with the publication of any data collected.

Go for 8 Wellness Program provides wellness encouragement in an informational and educational manner, with information that is general only and not specific to you, the reader. The contents of the program are intended to assist you and other readers in wellness efforts.
